I, ________________ (the Client) desire to engage Smarter Than Medicine (the Consultant) to provide certain services in the area of consultant’s knowledge and the consultant is willing to provide such services to the client.
WHEREAS, Kristin Ellison, (Smarter Than Medicine) is a current graduate student with advanced knowledge in “Nutritional needs, supplements, and various medical sciences and information. Smarter Than Medicine services do not guarantee results as results may vary. All consultations are based on information provided to Smarter Than Medicine by the client. Failure to disclose any/all but not limited to medical and recreational information may yield in undesired results. All information shared between client and nutritionist will remain confidential unless consent is provided.
Consultations are appointment only and are non-refundable. Clients have up to a 20 minute grace-period, and will be subject to the time scheduled and a late fee. Example: If a client schedules an appointment from 9:00 am to 10:00 am. If the client is 20 minutes late, then the client will receive a 40 minute consultation and a late fee. If a client needs to reschedule the appointment, the client must reschedule prior to 24 hours of the appointment. Rescheduling an appointment within 24 hours of the scheduled appointment will result in a service fee.
The client and consultant hereby agree to the following: the client agrees to comply with the all terms set by this agreement and will pay Kristin Ellison, (Smarter Than Medicine) all fees for consultations and services upfront. Clients will receive a signed copy of this agreement for their records once completed.
